PAYMENT TERMS
Net 30 days. Mada Medical Products reserves the right to assess a late charge of 1.5% per month on balances past due.

PRICES
Prices are subject to change without notice. All ground freight is paid by Mada Medical Products, Inc. on orders of $1,500.00 or more, shipped within the continental U.S. This excludes Infection Control products, Cylinders, noted carts & stands, nebulizers, steel cylinders, hazardous material, fuel surcharges, delivery appointments, lift gate, and inside delivery. There is a minimum charge of $50.00 per invoice and prices can not be less than Class 1.

ORDERING

RETURNS
No merchandise may be returned without a return goods authorization number from Mada Medical Products, Inc., Carlstadt, NJ. All requests for such authorization should include customer’s purchase order number and/or a Mada International, Ltd. invoice number with a reason for the return. All returned goods must be sent freight prepaid. Credit will be issued to your account on an invoice.

Do not credit your own account when issuing checks. Where there has been no error on Mada International, Ltd.’s part, authorized returns will bear a 20% restocking fee. Special orders, discontinued products, merchandise over 60 days old, and merchandise not in original cartons, will not be accepted for returns.

When ordering oxygen cylinders or oxygen kits filled, subtract the suffix “E”.

Mada cannot ship filled oxygen cylinders or oxygen kits into the following states, due to state regulations:
Filled oxygen cylinders, filled oxygen kits, and infection control products cannot be shipped by air.
